  • $67.24 - $76.36 an hour + Super

  • Open to Psychologists, Social Workers, Mental Health Nurses, Occupational Therapists and Counsellors
  • Position available in Hobart, Launceston, Devonport, OR Burnie office
 
ACM Healthcare is working on behalf of a Federal Government Department to source an experienced Mental Health Professional for a Clinical Care Coordinator role, providing Case Management services to Veterans and their Families. This is a two year contract position. 
 
Duties:
  • Deliver high-quality, evidence-based clinical services, including risk assessment, brief interventions, care coordination, and referrals for clients with complex/multiple needs.
  • Provide short-term, goal-oriented, clinical case management intervention to clients who present with unmet health and psychosocial needs that impact on their ability to access and/or meaningfully engage in mental health care.
  • Maintain accurate and confidential client records, including progress notes, care plans, and risk assessments.
  • Collaborate with internal teams and external organisations to ensure coordinated, client-centred support and effective referral pathways.
  • Work within a stepped-care model, supporting vulnerable or at-risk communities and families.
  • Participate in professional development and supervision to maintain registration and uphold best-practice standards.
  • Contribute to service improvement initiatives, program delivery, and community engagement activities as required.
 
Mandatory Requirements:
  • Qualifications as a registered Psychologist / Mental Health Occupational Therapist / Mental Health Registered Nurse with full AHPRA registration, OR
  • Qualification as a Social Worker with membership of the Australian Association Social Workers (AASW) on commencement OR
  • A Masters-level counselling qualification and registration with PACFA as a Registered Clinical Counsellor or with ACA at Level 3 or Level 4
  • Working with Children (WWC) and/or Vulnerable People Checks/Clearance must be obtained prior to commencement.
  • As this role is within a Federal Government department it is a required to be an Australian Citizen at time of commencement
